Reportedly, the individual in question was diagnosed with the rare 17α-hydroxylase deficiency within congenital adrenal hyperplasia, leading to a misalignment in gender identity, living as a female for 27 years.

Congenital adrenal hyperplasia is an autosomal recessive genetic disorder, where both parents carry the recessive genes, resulting in a one in four chance of having an affected child with each pregnancy.

Due to this condition, the individual couldn't produce sufficient male hormones to develop typical male characteristics, resulting in ambiguous or more female-leaning physical traits.

They never menstruated, and their breasts didn't develop. Hormone levels and progesterone results were abnormal at age 18, prompting doctors to suggest chromosomal testing, which was overlooked at the time.
